Acuvue® Contact Lenses from J and J Vistakon


Johnson and Johnson's contact lens division, Vistakon, was not a big time player in the contact lens field until they came up with the concept of disposable contact lenses, which they started marketing in the United States in 1989 under the name Acuvue®. Now they make and sell more contact lenses than anybody, with one and two week disposable contact lenses, one day disposable lenses, disposable colored lenses, and lenses for astigmatism and bifocals (which are not available in color). We used the original Acuvue® and Acuvue 2® Contact Lenses in the office for years because of their comfort and clear vision. Nearly everyone who wore the original lens has been switched to newer Acuvue® Oasys™ lens designs and materials, although some prefer the original material, still available in Acuvue®2. All Acuvue® contact lenses filter UV light. Many Acuvue® products have manufacturers rebates when an annual supply is purchased. Before 2004 all of Vistakon's lenses were made of the same hydrogel material (etificon A), which is 58% water. In 2004 the company received FDA approval for a new higher oxygen permeable silicone hydrogel material (galyfilcon A) called Acuvue Advance® with Hydraclear,. updated in 2010 to Acuvue ®Advance Plus. In 2005 Vistakon received approval for its most breatable silicone hydrogel material ACUVUE® OASYS™ with Hydraclear Plus(senofilcon A) which is promoted as even a better lens for dry eyes. It has FDA approval as a one week extended wear contact lens, and is recommended for 2 week replacement. Many patients have found the ACUVUE® OASYS™ lenses to be more comfortable than other contact lenses, although they are more expensive than Acuvue 2® Contact Lensesand Acuvue ®Advance Plus..
The first silicone hydrogel contact lens to correct astigmatism that was ACUVUE® ADVANCE™ for ASTIGMATISM. In June of 2008 Vistakon launched an astigmatism version of its most comfortable material when it created ACUVUE® OASYS™ for Astigmatism. Many people find these lenses more comfortable and stable than other contact lenses that correct astigmatism. In 2009 Vistakon launched a multifocal in the Oasys™ material--ACUVUE® OASYS™ for Presbyopia.
In the original Acuvue® material there are two groups of colored contact lenses. Acuvue® 2 Colours Enhancer contact lenses have a more intense color tint than the light visibility tint of the other Acuvue lenses, but only make a color appearance difference when warn on the eyes of people having a light blue or gray iris. They are available in 3 colors: Ocean Blue, Aquamarine and Emerald Green. Acuvue® 2 Colours Opaque contact lenses have irises painted on the lens and are available in more colors. They will change the appearance of eye color on people with any iris color, including dark brown and are available in 7 colors: Sapphire Blue, Deep Blue, Pearl Gray, Jade Green, Hazel Green, Warm Honey and Chestnut Brown.
All the lenses above are considered to be 2 week contact lenses available in packs of 6 lenses (Acuvue ®Advance Plus is also available in packs of 24).
There are three Acuvue® brand contact lenses available for the ultimate in disposability--one day. Acuvue® 1-Day Disposable contact lenses have been available for several years and, unlike the 2 week Acuvue® contact lenses, which are available in packs of 6 and 24 contact lenses, the Acvuue® 1-Day disposable contact lenses come in packs of 30 and 90. In 2007 the company released another daily disposable, Acuvue® 1 Day Moist, In 2010 the company released a third 1 day disposable material, 1•DAY ACUVUE® TruEye™ Brand, the first silicone hydrogel 1 day disposable.

Bausch and Lomb ( www.Bausch.com) is one of the oldest vision care companies in America. Although B&L no longer makes Rayban sunglasses (that was sold to Luxottica Group years ago), nor the eyeglass lenses, and optical instruments the company was famous for, it was one of the first companies to offer soft contact lenses in the earlier 1970's and many of the company's contact lens designs are among the best. It is also in the pharmaceutical business, and fell on hard times when one of its products,ReNu Multipurpose Contact Lens Solution with MoistureLoc, became involved with an outbreak of fungal kerititis. The product was taken off the market in May, 2006, but not without severely tarnishing the companies image and profits. Despite this we find that they continue to make several excellent products.
In the summer of 2005 Bausch and Lomb started to make available a silicone hydrogel called PureVision® lenses, which transmits 4 to 5 times more oxygen than most earlier soft contact lens materials. It is in the same class as Ciba's Night and Day® contact lens and, like the Night and Day™, has been approved by the FDA for 30 days of continuous wear. Although people sometimes find that the stiffer silicone based soft lenses are less comfortable initially, the long term benefits seem to favor these materials. The PureVision® contact lenses are less expensive than Night and Day®, but slightly more than the older soft contact lens materials.An updated more comfortable version of PureVision ® became available in 2011: PureVision ®2 HD. Rebates available for the PureVision® lenses, the comfort and vision of these lenses, and the aspheric design (which may add to visual acuity) makes this lens an attractive option for those wanting both extended (continuous) wear and daily wear of their contact lenses. The PureVision® Toric contact became available in December, 2005. It is the only toric contact lens approved by the FDA for up to 30 days of continous wear. It is based on the design of Bausch and Lomb's successful Soflens® 66 Toric. Bausch & Lomb also makes a 1 day disposable lens Soflens® Daily , which releases a moisturizing agent on each blink (similar to Ciba's Dailies contact lenses) and is competitively priced in boxes of 90 lenses. There are several rebates available on multiple boxes of the these lenses. With rebate an annual supply of lenses costs about 75 cents per day (37.67 cents per lens)!

CibaVision Contact Lenses and Other Products

CibaVision, a division of Novartis, has an array of eye products, including contact lenses and contact lens supplies. In 2011 the company was bought by Alcon.
Years ago Ciba bought another large contact lens company, Wesley Jessen. Wesley Jessen was the first company to popularize colored cosmetic contact lenses--contact lenses made to change appearance of the iris color of the eye. This includes their FreshLook® line of contact lenses and others. Although these lenses are legally only available by prescription, they have somehow made their way into flea markets and uncontrolled street vendors in large cities. Dr. E wonders about the motives of Ciba for allowing their cosmetic lenses, which can produce eye infections and need every bit as much care as other contact lenses, to be made available in such an uncontrolled way.
Wesley Jessen had several contact lens designs for astigmatism, including Torisoft®, DuraSoft® 2 OptiFit® Toric, available as long term daily wear contact lenses that come in a vial. Many of these designs worked well but by 2011 Ciba discontinued all of its vialled products, despite the fact that they worked and there were no other products that replaced them. They also made and discontinued a successful Toric Multifocal lens. Ciba called its first line of disposable and frequent replacement contact its Focus ® series. More recently it brought out the silicone hydrogel AirOptix series. One of the first disposable contact lenses available was a two week disposable lens called Newvue. It was similar to the original Acuvue contact lenses. Ciba changed the name of the Newvue to Focus® 1-2 Week. It, as well as the Focus® Monthly contact lenses are still made, but have never been as popular as some of the other disposable contact lenses. The Focus Toric Contact Lens was one of the first monthly (i.e. CL removed daily and replaced monthly) disposable contact lenses to correct astigmatism. It was discontinued in favor of Dailies ® Toric and AirOptix® for Astigmatism (see below).
Ciba has a couple one day disposable contact (lenses disposed of after a single days use) to compete with 1-Day Acuvue®. Focus® DAILIES®. with AquaRelease™ are made with a wetting agent that releases moisture throughout the day. It is only the only smaller diameter (13.8 mm) single use contact lens and we expect Ciba to discontinue it as they seem to do with all their products that work. In July, 2008 Ciba launched another one day disposable, DAILIES® Aqua Comfort PLUS, which is a 14 mm diameter lens, which releases twice as much water as the original DAILIES® with AquaRelease™. Focus® DAILIES® with AquaRelease™ and DAILIES® Aqua Comfort PLUS are available in only 1 curve, but unlike 1-Day Acuvue® contact lens which are available in 2 curves but only in corrections for myopia and hyperopia the Focus® DAILIES® (in its original material) are available in limited powers for astigmatism (Focus® DAILIES® Toric) and presbyopia (Focus® DAILIES® Progressives). Ciba's most innovative lens, and another way to possibly avoid contact lens cleaning and storage problems, is the silicone hydrogel Night and Day ® contact lens, approved by the FDA for 30 days of continuous wear (see Characteristics of Silicone Hydrogel Contact Lenses). It transmits more oxygen than any other lens material. Oxygen transmission is measured as units of Dk/t and Night and Day has a value of 175 Dk/t, making it 8 times more oxygen permeable than earlier soft contact lens materials. In 2009 the name and material was changed to AirOptix Night and Day ® Aqua. It is available in myopic powers up to -10.00 diopters and hyperopic powers up to +6.00 D in two base curves (8.6 and 8.4) and one diameter (13.8). In our office some of the earliest successes we have had with these lenses were not people who were trying to achieve long term overnight wear with lenses as much as people who had dry eye problems who found the low water content of the material (24%) conserved corneal tear film better than high water content contact lenses, or people who need a smaller diameter contact lens. Night & Day Contact Lenses are not available in a toric design for astigmatism. Whether one wears the lenses continously or removes them, Night and Day Contact Lenses are considered to be 30 day disposable lenses, and because each box contains 6 lenses, a year supply is 4 boxes, similar to the only other FDA approved 30 day continuous wear contact lens, Bausch and Lomb's Purevision. Even with the higher oxygen permeability, not everyone can tolerate contact lenses overnight on a regular basis. People with allergies and who are in dirty environments still may need to remove and clean contact lenses.
Ciba makes three other highly oxygen permeable Silicone Hyrogel contact lenses: O2Optix®, designed for 2 week wear, AirOptix® Aqua Spheres, AirOptix® for Astigmatism, and AirOptix® Multifocal, all designed for monthly replacement.
Two unique and effective contact lens care systems which were made by Ciba are no longer available: AOsept, a hydrogen peroxide based disinfection system (which Ciba acquired when it bought American Optical contact lens division in the 1980's ) and Quickcare, a two bottle care system which cleaned and disinfected contact lenses much faster and better than any of the more popular single multipurpose solutions. Although AOsept solution is still available, the cleaning component of the system isn't. It has generally been replaced by Clear Care®, which is the same AOsept peroxide solution, but without the separate cleaner. For people who are allergic to other multipurpose contact lens solutions it may be the best choice, but one must remember to use the special case that comes with the solution to neutralize the peroxide. prescription lenses for both men and women. Their IMF magnetic clip-on frames have polycarbonate
polaroid clip lenses that fit on their metal frames, including titanium, and also plastic frames. Colors of
the sunclips match the frames. In 2005 they also came out with a "bling-bling" look and also a
"bling-bling-bling" look. These are jewelled clips and jewelled clips that fit on matching jewelled frames.

Continous Wear Contact Lenses--Silicone Hydrogels

The newest category of contact lenses is silicone hydrogels. The main advantage of using silicone in a contact lens is that it transmits much more oxygen than any other element used in contact lens materials. The disadvantage is that silicone is hydrophobic--it does not absorb water at all. A lens made of pure silicone is very uncomfortable. It must be combined with some other material to make it hydrophilic ("water loving") and that is what has been done in such continous wear contact lenses as Ciba's NIGHT & DAY® and Bausch & Lomb's PureVision™. It is also used in Ciba's O2OPTIX™ and Vistakon's ACUVUE® ADVANCE™, ACUVUE® ADVANCE™ for ASTIGMATISM, and ACUVUE® OASYS™ materials. Silicone has been used as a component of gas permeable rigid contact lenses for at least 20 years. Combining the silicone elastomer, or rubber, to the soft lens, or hydrogel material, was not easy for chemists to do. It has been likened to combining oil and water, but the task was accomplished by 1999, by chemists at competing companies using competing techniques.
Once they had developed the new silicone hydrogel materials the companies designed and tested new contact lens designs. Only Ciba's NIGHT & DAY® and Bausch & Lomb's PureVision ™ contact lenses are approved by the FDA for 30 days of continuous wear. That means that people can wear them 24 hours per day for a month without removing them for cleaning and disinfecting, something that needs to be done with all other contact lenses. Not everyone can wear them in this manner. But for those people who can, it provides an alternative to refractive surgery such as LASIK, epecially for those who are poor surgical candidates, such as people whose eyes are changing. A charactertic of silicone hydrogels is that these materials are stiffer than traditional soft contact lenses or hydrogels. This stiffness can be measured in the laboratory as the "modulus of elasticity." An article written October, 2005, and available on the Internet by Eric Papas PhD describes this: Elastic Modulus and Silicone Hydrogel Contact Lens Fitting . Because of this stiffness factor, some people who were wearing traditional soft contact lenses find that the new silicone hydrogel contact lenses are initially not as comfortable as their previous soft contact lenses. Yet others find that the silicone hydrogels are much more comfortable, and most find that they are more comfortable over the long term. Because of the modulus of elastitcity the base curve of silicone hydrogels is more critical than in other soft contact lenses. Unfortunately many silicone hydrogels are available in only one base curve, especially the toric designs, and none are availble in more than two base curves. Of the available silicone hydrogels NIGHT & DAY® has the highest modulus of elasticity (i.e. it is the stiffist) and ACUVUE® ADVANCE™ has least modulus of elasticity (is the softest). Of contact lens brands approved for continous wear we prefer the PureVision ™. Most people report it is more comfortable than the NIGHT & DAY® contact lenses. It has a lower modulus of elasticity than NIGHT & DAY®. We find that often people who find the NIGHT & DAY® very comfortable find the PureVision ™ to be uncomfortable. This might be due to available diameter, base curves, or other factors. The more materials and the more base curves to choose from, the more likely we are to find a lens for a difficult patient. A couple of reasons why the PureVision ™ is often the preferred continuous wear contact lens, besides the comfort factor, it is aspheric , which reduces spherical aberration and tends to provide better clarity than lenses that are not aspheric, and it happens to be the least expensive 30 day continuous wear contact lens. When people are wearing contact lenses in the 24 hour per day continous, or "extended wear" mode, it is important to have the cornea examined regularly with a slit lamp biomicroscope by your eye doctor. Earlier soft contact lenses had been approved by the FDA in the 1980's to be worn overnight, some for 30 days. A certain percentage of people developed keratitis and corneal ulcers with these earlier lenses and people were cautioned not to wear contact lenses overnight (although many still did, without problem). The reason for the corneal ulcers was thought to be from hypoxia, a lack of oxygen getting to the corneal cells when the contact lenses were worn. The new silicone hydrogel materials transmit several times more oxygen than the earlier extended wear contact lenses. Rigorous testing studies have been done in various locations around the world, using both the PureVision ™ and NIGHT & DAY® contact lenses, having subjects wear the contact lenses continously for a month at a time. The results showed that adverse events were minimal, and the FDA has approved both of these contact lenses for 30 days of continous wear. Ciba's patent infringement lawsuit against B & L prevented B & L from making or selling PureVision ™ in the United States until the summer of 2005. B & L has added a toric lens version of the lens PureVision Toric™, to correct astigmatim, which became available in December, 2005 and multifocal version, called PureVision Multifocal™ to act correct presbyopia. ACUVUE® OASYS™, Ciba's O2OPTIX™, as well as Ciba's Air Optix for Astigmatism are approved as 6 day extended wear contact lenses.

CooperVision is one of the largest contact lens manufacturers in the world. They make several designs of toric soft contact lenses for astigmatism. The company acquired the California based Ocular Sciences, which makes several brands of disposable soft contact lenses. The most popular in the former Ocular Sciences line are Biomedics® 38 and Biomedics® 55 , the number indicating the water content of the lenses. The material of Biomedics® 38 is Polymacon, the same material in Bausch and Lomb's Soflens® 38, and one of the first materials used in soft contact lenses in the 1970s. Some people, especially those with dry eyes, found those earlier materials to be more comfortable than silicone hydrogels, which became available two decades later. Coopervision launched it first silicone hydrogel contact lens material in 2008 Avaira™, a 2 week disposable lens which has special wetting properties. Later Biofinity® and Biofinity® Toric became available, the latter of which has some of the largest parameters in silicone hydrogels to correct astigmatism. The silicone hydrogel conttact lenses transmit more oxygen and are approved for 1 week of continous wear. .Over the years the specialty of CooperVision has been special order toric contact lenses for astigmatism. Many of these designs are needed for patients with high amounts of astigmatism and were were originally made by another California company that CooperVision acquired, CoastVision. None of these lenses is made in Polymacon. Brand names of these lenses for astigmatism are Hydrasoft Toric, Preference® Toric, Proclear® Toric, Frequency® 55 Toric, Biomedics® Toric and Biofinity® Toric . Although the specially made toric lenses are individually made, and put in glass vials after fabricated, even the custom torics are now available in groups of 4 glass vials, at a cost close to what one of these contact lenses cost. That is, you might pay $150 for one lens, or $175 for 4 lenses. They consider the lenses in the 4 pack to be a 4 month, or Quarterly, disposable, but it is the same lens as the one in the single bottle. Marketing conditions--the trend in the ophthalmic field towards disposable contact lenses and poorer available contact lens care products (see Dr Elman's All About Contact Lens Care products) has driven the company to do this. In general the CooperVision soft toric contact lenses are still more expensive than other brands, but sometimes they make the only parameters that are available for patient's with high amounts of astigmatism or unusually shaped corneas.

Shuron has been an optical manufacturer since 1865. What is amazing is that many of the frames that they made
60 or 70 years ago they still make. Wow! Talk about classics. This makes them the king of the retro look. These
are the frames your grandparents wore--and we know some people get them in pawn shops, yard sales, and flea

stiff temple that hugs the head but is not as tight [as pictured on frames to right below]). Most modern day frames
do not have these options.

In general the categories are arranged by contact lens diameter and whether the lenses will have a prescription (Rx), or not have prescription (plo). The larger diameter contact lenses (18 and 22 mm) are more expensive than the more traditional diameters of 14.5 and 15 mm.

Note that not all lens designs are available with prescription and in general the lens powers are restricted to myopic powers up to -6.00 dioptors. Toric contact lenses for astigmatism and plus lenses to corrected hyperopia are not available. These contact lenses are not meant to be worn for as long as contact lenses that correct vision and materials are traditional hydrogel--not the newer silicone hyrogels. Silicone hydrogels resist the dyes necessary to make theatrical lenses. The Custom Theatrical designs all cost more than the ready made Gothica® styles and take longer to get (expect 2 weeks).
